As of October 11, 2021, The Medical Practice formerly operated under the name of "Sundance Medical Associates" and located at 633 East Ray Road, Suite #133 in Gilbert, Arizona (85296) was acquired by Optima Medical.
All aspects of this former medical office/practice including patient records and responsibility for ongoing patient care was tranfered to their custody as of that time.
