We appreciate the confidence you have shown by choosing our office to provide medical care to you and your family. We make every effort to give you the best possible care. In order to achieve this, we ask for your assistance in the following clinic policies.
Appointments
Appointments are required to address any/all issues and concerns. Regular office hours are from 9 AM to 5 PM with lunch taken between noon and 1 PM (phones generally not answered during lunch). We reserve some slots for same day “sick” visits every day. We work hard to accommodate requests for same day appointments, but are not always able to do so. New patients must arrive 30 minutes prior to their appointment time. If you are an established patient, you must arrive at least 10 minutes prior to your appointment time. If you arrive late, we may need to reschedule your appointment.
If you are unable to keep your scheduled appointment, please call our office 480-782-0609 at least 24 hours ahead to cancel so that your appointment time can be made available to other patients. Sundance Medical Associates PLLC reserves the right to bill $50 for No-Show appointments (Appointments Missed without advanced notice of any kind). A lessor fee of $25 may be assessed for failure to provide adequate notice of cancellation (24 hours). Multiple violations of these office appointment policies (No-Shows/Missed Appointments/Late Cancellations) may result in a patient being discharged from the office.
Test Results
It is the policy of this office that all diagnostic testing (labs, imaging studies, etc.) will ideally be reviewed within the framework of a planned follow-up appointment. There are times when you may be directed to call the office to check on your results by phone. Often, our providers or medical assistants will contact patients with abnormal test results to ensure appropriate follow-up arrangements are made. However, in all cases where diagnostic testing is performed, it is ultimately the patient’s responsibility to ensure they are in receipt of the results and understand the treatment plan. *Do not ever assume “No News is Good News”. If you have not been provided the results of any diagnostic testing performed outside our office within 7-10 business days of having completed the test/exam, please contact our office. If you have a scheduled follow-up appointment to review testing results, you can safely wait until that time unless you are advised differently by a member of our staff.
Medication Refills
We require up to 72 hours notification for all refill requests. Please do not allow yourself to run out of medicine. Your provider requires advance notice, as he/she needs to evaluate your medical records. Narcotic medications will NOT be refilled on weekends or after normal office hours. Please request medication refills through your pharmacy.
Referrals
All referrals require an office visit. When your provider wants you to be referred to a specialist, it can take up to 10 business days for completion. If it is an urgent referral, it will be completed within 48 hours. You will receive all the information you need by mail or phone so that you are then able to call and schedule your appointment. If your specialist requires additional visits, it is your responsibility to contact out office 10 days prior to your next appointment with them.
Medical Records
In order to release any medical records, we require a properly formatted Medial Records Release form which must be completed and signed by the patient. SMA can provide you with this form when/if needed. Please allow up-to 15 business days to process your records request. Medical Records may be faxed to another physician's office OR made available for you to pick-up in our office.
Please note: Medical Records required for the purposes of continuing medical care (to another physician/provider) are generally provided at no cost to our patients. However, all other requests may be subject to nominal charges to allow for the required staff time, supplies, and reproduction materials which are not a part of our operating model.
Insurance
It is the patient’s responsibility to understand their insurance plan and its benefits. We will try to assist you when possible with what is and is not covered. The patient is responsible for services/items that the insurance does not cover.
Payments
The patient is responsible for payment of each office visit. If you have insurance, our billing department will file the claim with them as a courtesy. Please have your insurance card with you at each visit. Co-payments and deductible payments are always required on the day of service. If you do not bring your payment with you, you may be asked to reschedule your appointment. In the event that you do not have insurance, payment is due on the day of service.
Please pay with cash, credit or debit card. We do not accept checks. Patients without insurance will need to pay for their service on the day it is rendered. A late fee of $50 is assessed on ALL unpaid balances after 60 days.
After-Hours Emergencies
If you have a true medical emergency, please call 911 or go to the nearest emergency room to receive medical assistance immediately. A physician is on call after hours for urgent needs that can not wait until the next business day. Medications will not be refilled after hours. Narcotics and antibiotics are not called or faxed to pharmacies after hours.
